APPLICATION REQUIREMENTS
BIRTH CERTIFICATE

My name is not readable on my birth certificate, what should I do?
You must submit additional documents proving your identity, where your name is clearly written. These are some of the acceptable additional supporting documents you may submit together with your original birth certificate:
Birth Records issued by the Municipal Local Civil Registry; or
Certified-True-Copy of your Passport
Do I still need to request a PSA-authenticated birth certificate even if I already have an original NSO-authenticated birth certificate?
NO. The original NSO birth certificate is accepted.
Does the birth certificate need to be authenticated by DFA?
NO.

MARRIAGE CERTIFICATE
​
I am already married but the name in my passport is still under my maiden name. Will I still be required to submit an original marriage certificate?
Yes. The Board of Nursing requires all applicants to declare all legal names the applicant has used.
Will I be allowed to use my maiden name in the NCLEX application form even if I am already married but my passport is still under my maiden name?
Yes. You will be required to submit your original birth and marriage certificates.

PASSPORT
​
Will the Board of Nursing process my NCLEX Application even if I do not have a passport yet?
Yes. But you must have your valid, unexpired passport at the time of your NCLEX-RN exam. Passport is the only acceptable proof of identity by Pearson Vue Testing Center at the time of your exam. Failure to present the original passport will be refused entry to the testing site.
What will happen if I forget to bring my passport on the day of my exam?
You will not be allowed to take the exam since a passport is the only acceptable document to prove your identity.

OFFICIAL BSN TOR

Can I submit an old original BSN Transcript of Record issued by my nursing school?
The BON requires another form to be completed & and signed by your nursing school.
It is necessary that the issued date on your TOR is the same as the date on the BON CRLE form, therefore you will need to request a new BSN TOR.

CERTIFICATION OF RELATED LEARNING EXPERIENCE (CRLE)

Where will I get this CRLE form?
PNICare provides this form to you together with the BSN Request Letter that you may use to submit to your nursing school. You will receive this by email but should you require a hard copy of this document, PNICare will mail it to you.

My nursing school did not specify the hours and weeks in the listed subjects in the CRLE form. Will the Board of Nursing accept this?
NO. The Board of Nursing will issue a deficiency notice and will require a new CRLE with specific hours and weeks for all subjects to be submitted.

We recommend that you inspect the CRLE form before your nursing school seals this document in the envelope.
HIGH SCHOOL FORM-137
​
What is Form-137?
The Form-137 is also called the Student Permanent School Record. This document lists the subjects and grades from when you started High School until graduation.

My school does not have an official envelope, what will I do?
Submit the original HS Form-137 to PNICare and we will put it in a white envelope.
​
My school has closed and is no longer operating, where should I get this document?
You may request this document from the Department of Education.
​
What options do I have if I cannot provide my original Form 137?
You must inform PNICare if this is the case. We might have to explore processing your NCLEX application to another board of nursing.

CERTIFICATE OF EMPLOYMENT (CoE)
​
Can I submit a certified true copy of my CoE if I am not able to submit the original document?
Yes.
​
How do I process a certified true copy of my CoE?
Present the original and photocopy of your CoE to a Notary Public who will certify that the photocopy presented to him/her is the Certified-True-Copy of the original.
I have zero nursing experience, will I the board of nursing accept my application for NCLEX?
Yes. You will be required to complete an online Refresher Course by the Board of Nursing. PNICare will handle your enrollment in this refresher course which can be completed within 5-10 days.
My nursing experience is less than 5 years in the last 5 years. What will I do?
If your experience is 3 years or more in the last 5 years, submit the original or certified true copy of your CoE/s to PNICare. Be ready to be required by the Board of Nursing to complete an online refresher course.

Is my experience in Telehealth or (Healthcare)BPO accepted?
The NMI Board of Nursing only accepts nursing experience in hospitals/clinics/OPD or any settings where direct patient care is rendered.
